What the inspector wrote

On October 8, 2024, Licensing Program Analyst (LPA) Kris Diaz conducted an unannounced visit to follow up on a UIR reported to PRO on 8.30.24. LPA was greeted by assistant, Selena Garcia who granted LPA access to the facility. Licensee was completing school pickups and arrived shortly after LPA arrival. The incident occurred at the facility on 8.29.24. Currently residing in the home are the Licensee, her spouse, and their minor children (11,14). LPA observed 12 children in care with two staff. Licensee arrived with two more children. Description of the incident : A incident occurred on 8.29.24. Per licensee, C1 often wanders away from the group to do their own activities. On this date during storytime licensee was singing to the children when C1 walked less than 5 feet from the group and got a chair and climbed on top of it. C1 put the chair in the center of the room under a light that had a sticker resembling the world map. C1 stated they were trying to jump and touch the light when the chair fell backwards and C1 fell with it. C1 sustained a break on their arm. Licensee stated she did not observe the fall and C1 did not cry or indicate they were injured right away. Per licensee, C1 walked up to licensee and stated \342\200\234my arm.\342\200\235 Licensee stated C1 walked around for about one minute and as she observed C1 she noticed redness and swelling of C1s arm. Licensee put ice on the arm and contacted C1s mom. C1s mom asked licensee to take C1 home to their father. Licensee took C1 home. Based on the information obtained, it is determined that the Licensee provided proper care and supervision and rendered aid to child, contacted parents, and reported to licensing in a timely manner. No deficiencies were issued during this visit. This inspection was conducted in person. A copy of this report was read and left with licensee\342\200\231s assistant, Selena Garcia. Licensee left to complete after school pickups. A Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted.
