What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Case Management inspection to the above facility on 12/17/2025.  LPA arrived at the facility at 8:30 AM, identified self and met with Ipalawatte Sunethra, Director who guided LPA on a tour of the facility. There were 4 infants present with 1 staff during today’s inspection. During the complaint inspection conducted on October 10, 2025, LPA observed staff member (S1) caring for infants without having completed the required units. In addition, the facility did not have the necessary licensing forms, mandated reporter training, or proof of immunization's for the staff. As discussed during a previous visit, the LPA reminded the Director that unqualified staff may not be left alone with infants. The Director explained that she is typically the staff member present with the infants between 8:00 AM- 9:00 AM until the other teacher arrives. Upon arrival, LPA observed the Director in the infant room. At 9:10 AM, LPA reviewed the required paperwork for S1 and observed for enrollment for Early Childhood Education course but did observe proof of immunization's and a completed mandated reporter training. The following deficiency listed on the attached deficiency page (LIC 809D) is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Ipalawatte Sunethra and Appeals Rights provided.

  • ARTICLE 1. General Provisions and Definitions

    (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement was not as evidenced by: LPA observation, interview, and record review of S1 staff file. This poses an potential health and safety risk to children in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    (b) (1)   On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement was not as evidenced by; LPA observation, interview, and record review of staff file. This poses an potential Health and Safety Risk to children in care.
