What the inspector wrote

On August 28 2024, a child in care was injured while interacting with his/her classmates. The injuries he sustained required the child to be seen by medical personnel the same day. The child's mother notified the school via email immediately, that same evening, after returning from seeking medical care for the child. Per title regulations, Upon the occurrence, during the operation of the childcare center of any of the events specified, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours of any injury to any child that requires medical treatment. Based on information obtained which included document review, interviews with parents and staff it has been determined that Title 22 Reporting Requirement was not met. A Type B Violation of the California Code of Requirement was not met. A Type B Violation of the California Code of Regulations, Title 1201212(d) (1) (B) is cited on the attached LIC 9099- D.
