What the inspector wrote

On 09/22/2025 Licensing Program Analyst ( LPA) Doris Whitmore , conducted a case management to follow up on an Unusual Incident, reported to the department by telephone on 09/12/2025. LPA met with Site Supervisor Cynthia Navarro and toured the facility. LPA observed 10 children and 3 teachers. According to the Unusual Incident child 1 was putting a tile down that children were jumping on them. Child 2 jumped on ( C1's) right arm. Staff 1 and Staff 2 rendered aid to the child1. Staff contacted child 1's parents. Staff 1 stayed with child 1 until child 1's parents arrived. Staff gave child's parent ouch reporte that she signed. RP stated she is waiting to confirm if child 1 was taken to the doctor, During the investigation LPA interviewed ( S1) ( S2) ( C1) and ( C2) LPA obtained a copy of the Ouch Report. ( S1) was sitting in the large carpet with ( C1). ( C1) was kneeling next to ( S1) when ( C2) jumped on the tile. ( S2) was in the housekeeping area. LPA concluded this case management incident. No Title 22 Violations have occurred, no deficiencies cited, and no Lapse of Care and Supervision. During the interviews there were no disclosures. A notice of Site Visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor Cynthia Navarro,

This requirement was not met as evidence ( S2) disclosed that( C1) went home with just shorts and no diaper on.
