Inspector’s narrative

What the inspector wrote

On 09/22/2025 Licensing Program Analyst ( LPA) Doris Whitmore , conducted a case management to follow up on an Unusual Incident, reported to the department by telephone on 09/12/2025. LPA met with Site Supervisor Cynthia Navarro and toured the facility. LPA observed 17 children and 3 teachers. According to the Unusual Incident RP states that parent 1 came to pick up child 1 early. Parent 1 called the facility mins. later and spoke to RP. Parent 1 stated she noticed that child1 did not have a diaper or underwear under shorts when she arrived home. RP spoke to the staff in the classroom. Staff1 told RP that child 1 went to use the bathroom. Child 1 took her diaper off since it was wet. Staff 1 told child 1 to throw it away. Staff 1 asked child 1 if she wanted diaper and Child 1 stated no, Child 1 put her shorts back on and washed her hands. RP spoke to parent 1 regarding what happened and provided her with the information that staff 1 shared with RP. During the investigation LPA interviewed ( S1) ( S2) and ( C1). LPA obtained a copy of the Staff Meeting with signatures, restroom log, and ouch report. LPA reviewed the file of ( C1). Based on the information obtained and interviews conducted ( S2) acknowledged that ( C1) went home with just shorts and no diaper. ( S2) disclosed that ( C1) showed her the diaper.( S2) asked ( C1) did she want another diaper. ( C1) stated that she did not want another one( C1) ripped the side of the diaper and ( S1) told her to throw it away. deficiency is being cited in accordance to Title 22 of the California Code of Regulations and or Health & Safety Code. There was sufficient evidence to determine that Personal Rights Violation occurred. Please see 809 D An exit interview was conducted, a copy of this report, notice of site visit and appeals rights were provided to Site Supervisor Cynthia Navarro.

This requirement was not met as evidence ( S2) disclosed that( C1) went home with just shorts and no diaper on.
