What the inspector wrote

On June 12, 2025, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. The purpose of the inspection was to follow up on Plan of Correction (POC). LPA met with the facility representative who granted access. LPA toured the facility with the representative and observed forty two day care children and eight staff in care. All adults residing in the facility have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Index Clearance and are associated to the facility. On May 21, 2025, LPA conducted an annual inspection of the facility and observed an area in the play yard where the artificial turf was not secured to the ground and posed a potential tripping hazard to the children on the playground. Today, LPA arrived, inspected the area where the artificial turf was lifting and observed it was repaired and provided a safe area for the children. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, were provided to Shirley Johnson, Representative at the facility. A Notice of Site Visit (LIC 9213) must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
