What the inspector wrote

On August 01, 2024, Licensing Program Analyst (LPA) Andrew Alemoh conducted a case management inspection to conclude the Unusual Incident reported to the department by telephone and email on 07/10/2024; this incident was reported timely. LPA spoke with the Director regarding the incident. LPA toured the facility and took a census of the children. Upon arrival, there were 36 children and 8 staff present today at the facility along with the Director. Description of the incident: An incident on 07/10/2024, Director called to notify the department that a S1 was covering the infants heads with blankets while pushing their heads down during nap time. Based on the information obtained, it was determined that Staff #1 has been placing blankets to cover the infants head during nap time, along with pushing the infant’s heads down for sleep during nap time. The facility handled the situation accordingly by disassociating(terminating) Staff #1 from the facility. This is a Type A violation which shall be posted for 30 consecutive days as there is immediate risk to the health and safety of the children in care. and an immediate health and safety regulations. Also, LPA Alemoh informed the Director Shirley to provide a copy of this licensing report dated that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and a copy of this report was provided to the licensee along with appeal rights and the Notice of Site Visit.
