What the inspector wrote

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with Director Jeannine Pignotti.

    CRIMINAL RECORD CLEARANCE

    All individuals subject to a criminal record review...shall prior to working, residing or volunteering in a licensed facility:(1) Obtain a California clearance or a criminal record exemption as required by the Department. This requirement is not met as evidenced by: Based on record review and interview, one Staff (S1) worked at the facility begining in March 2025 but did not get a criminal record clearance until August 2025. This poses an immediate health, safety or personal rights risk to persons in care.
