On 03/25/2025 at 02:30PM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced Plan of Correction (POC) Visit. LPA identified self and met with Sonia Aguilar, Licensee. LPA identified self, reason for the visit, and toured the inside and outside of the facility. There were 5 children and 1 staff present. During today's visit, LPA cleared the Plan of Correction for the two (2) Type B Deficiencies cited on 02/25/2025 for 1596.8662(b)(1 ) Administration of Child Day Care Licensing an d 102421(b) Child's Records . LPA observed the Mandated Reporter Training Completion Certificates for Licensee and Staff #1, LPA observed that 6 of 6 children's files contained LIC627 Consent for Emergency Medical Treatment and provided Licensee with POC Cleared Letter. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Sonia Aguilar, Licensee including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
