On 5/14/2025, Licensing Program Analyst (LPA) Loyce Phillips arrived at above-mentioned facility for the purpose of conducting an unannounced Case management-other visit. Upon arrival, LPA met with Director Aracele Gonzalez and toured the facility. LPA observed 35 preschool children sleeping with 4 staff members. Description of the incident: On 2/24/2025, LPA observed a large refrigerator outdoors in the children's lunch area. LPA addressed the refrigerator with Director Araceli and stated that the refrigerator needed to be removed, as a refrigerator can not be located outdoors in the lunch area where children eat. The Director and Director of Operations had maintenance remove the refrigerator to the storage area of the facility. LPA conducted a walk-through of the facility with the Director, providing technical assistance and best practices on the physical plant. Technical advisories were discussed and provided to Director. An exit interview was conducted, a copy of this report, appeals rights, and notice of site visit was provided to Director.
