What the inspector wrote

On 10/14/25 at 8:40AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced,\302\240 Case management incident inspection. Upon arrival, LPA met with and were guided on a tour of the facility by Center Director Pam Youngblood to whom the purpose of the inspection was announced. There were 19 children being supervised by 6 staff. The incident was reported to Community Care Licensing on 09/30/25 via phone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident which occurred on 10/1/25 was Child #2 C2 fell on Child #1 C1 and C1 sustained a bruise to the right cheek. During the inspection, LPA obtained a copies of the facility roster, ouch reports, conducted interviews and took photos. On 10/1/25 around 8:45am, while the children were waiting to use the rest room, C1 and C2 was sitting on the bench with Staff #1 S1. Per S2 The children were sitting on both ends of the bench, while S1 sat in between them. C2 got up and started to run towards C1, lost their balance and fell onto C1 and bumped heads. S1 helped the child up and asked if they were ok, both said they were fine. Per S1 there was no visible signs of injury to either children at that time. Around 10am staff noticed a purple bruise on C1's right cheek. Staff put ice on the injury and reported incident to the parents via a phone call. Both children interviewed stated they were playing and fell. C1 stated his face hurt but they did not cry. The department finds there was no deficiency in compliance. Exit interview conducted and report was reviewed with Center Director Pam Youngblood .\302\240 A Notice of Site Visit was given and must remain posted for 30 days.
