What the inspector wrote

On 10/14/25 at 8:40AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced,\302\240 Case management incident inspection.\302\240Upon arrival, LPA met with and were guided on a tour of the facility by Center Director Pam Youngblood to whom the purpose of the inspection was announced. There were 19 children being supervised by 6 staff.\302\240 The incident was reported to Community Care Licensing on 10/14/25 via phone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. During the inspection, LPA obtained a copies of the facility roster, permission slips, zoning map conducted an interview and took photos. The center is reporting that Melvin Avenue school is installing new privacy fencing on the play ground and the children will not be able to use it during the installation. The installation began on 10/13/25 and is due to be completed by Friday 10/17/25. Per staff, Instead of playing on the playground, during their normal play time of 8:45am-9:30am, the children will go on nature walks, within the campus, to look at the trees and nature. The children will be supervised by 3 staff per class and they will follow the playground zoning map provided by the school. LPA observed the parents signed a permission slips informing them of the trips. Exit interview conducted and report was reviewed with Center Director Pam Youngblood.\302\240 A Notice of Site Visit was given and must remain posted for 30 days.
