What the inspector wrote

On 2/11/26 at 8:35AM, Licensing Program Analysts (LPAs) Jeanine Lipsey and Samantha Barba conducted an unannounced, in-person Case management incident inspection. The incident was reported to Community Care Licensing on 2/3/26 via phone.\302\240 The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident occurred on 2/2/26. Incident: A substitute teacher swung a child by the arm. Upon arrival, LPAs were guided on a tour of the facility by Director Pam Youngblood to whom the purpose of the inspection was announced. Census:\302\240 There were twenty-eight children being supervised by seven staff. During the inspection, LPA's obtained a copy of the facility roster, Employee roster, and conducted interviews with three staff, twelve children, collected copies of three staff statements and the Substitute teacher's information. This incident needs further information and will be concluded at a later date. Exit interview conducted and report was reviewed with Director Pam Youngblood.\302\240 A Notice of Site Visit was given and must remain posted for 30 days.
