What Happened

Complaint investigation substantiated unsupervised children incident. During 10-day inspection follow-up, LPA observed 1-2 minute periods with children unsupervised when teacher stepped away for restroom assistance while remaining children were left with hired music teacher with no additional staff available to maintain ratios.

Observations

  • Children left unsupervised for approximately 1-2 minutes while teacher assisted child with restroom break
  • Teacher stepped away from classroom leaving remaining children supervised only by hired music teacher
  • No other staff available to provide additional supervision due to requirement to maintain ratio
