What the inspector wrote

Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced annual required inspection at the child care center on 07/13/2022. LPA met with Teacher 1 (T1) at 12:34 PM. LPA toured the facility, to take census of children in care. LPA observed preschoolers napping on cots in two rooms with the proper teacher to children ratios. There is also an infant program with facility number 197493147 on premises. LPA observed toddlers napping in the infant program. The infant program has a toddler component attached to the infant license. Director Ava Brown arrived at facility at approximately 1:01 PM. All adults, working or volunteering at facility have a criminal record clearance or exemption. LPA was guided on a tour of the facility by Director, Ava Brown. Physical Plant Facility operation hours are Monday through Friday 7:00 PM to 5:00 PM. LPA observed all areas identified on the facility sketch. The facility was kept clean, neat and orderly. There are four properly working toilets and sinks in the preschool bathroom that are safe and sanitary. All poisons and hazardous items are stored in the kitchen under the kitchen cabinet and inaccessible to children in care. All floors are clean and safe. Kitchen was kept clean and free of litter, rubbish and rodents and/or any other vermin. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. Facility was observed to be free of flies, other insects and rodents. Outdoor playground equipment is in safe condition and is free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA did not observe any high climbing equipment, slides or similar equipment. There are swings in which all areas around or under the swings are cushioned with material that absorbs a fall. There is adequate shade in the play yard. PAGE 1 FACILITY RECORDS There is at least one person present at facility that is trained in Pediatric CPR and First Aid. LPA also observed that at least one staff has completed Mandated Reporter training. The name of the child care center director or fully qualified teacher(s) designated to act in the director's absence is on file. LPA was unable to review staff records during today's inspection. Per Director, key to access staff files was not available. LPA advised to ensure that all files (children and staff) are available upon request. Director plans to electronically file each individuals file electronically to ensure files are available upon Department's request. LPA observed parent board located in the main office of the facility with the following required postings: Facility License Monthly Menu: Children in care are provided breakfast, lunch and a snack PUB 269- Child Passenger Restraint System Poster PUB 393- Notification of Parents' Rights LIC 610- Emergency Disaster Plan LIC 613A- Personal Rights LPA reviewed the following documents during today's inspection: Verification of Disaster Fire Drills: Per Director and LPA review, fire drills are conducted last Friday of every month. Daily Activity Schedule Sign In/Out Sheets LPA provided Driector with the following form for review and completion: LIC 9148 Earthquake Preparedness Checklist PAGE 2 LPA reviewed the following children's records: LIC 613A- Personal Rights Admission Agreement LIC 700- Identification and Emergency Information LIC 701- Physician's Report LIC 995- Notification of Parents' Rights LIC 627- Consent for Emergency Medical Treatment Immunization Record The following was thoroughly discussed: Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. The facility was operating in substantial compliance during today\342\200\231s inspection on 07/13/2022. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. PAGE 3 Notice of Site Visit A notice of site visit was given and must remain posted for 30 days. Exit Interview Exit interview conducted and report was reviewed with Director, Ava Brown.
