What the inspector wrote

On 07/22/2025, Licensing Program Analysts (LPAs) Elicia Calvillo and Daniel Bombino conducted an unannounced 3 Year Annual inspection at 18161 Gresham Street, Northridge CA 91325. LPAS arrived at the facility at 09:00 AM, identified self and met with Liliana Cortez, Licensee, who guided analyst on a tour of the inside and outside of the facility. There were six (6) children and one (1) assistant present at arrival. LPAS provided Licensee with a copy of the LIC125 Entrance Checklist to help facilitate the inspection. Facility operation hours are Monday to Friday from 6:00AM to 05:30PM. The family child care home is one story dueling with a combination living /dining /kitchen /laundry room, three (3) bedrooms, two (2) bathrooms, and no garage. There is a front yard that will be used for dropping off and picking up children. There is a fenced outdoor play area in the back of the house. The children enter and exit the child care home from the front door. LPAs observed the living room area, bedroom #1 and bathroom #1 will be used for child care and accessible to parents and children. The living room area is used for indoor play, child activities, circle time, eating, and sleeping. LPAs observed toys and play items that are safe clean and appropriate for the ages of the children. LPAs observed three (3) cots and bed linens and two (2) pack n\342\200\231 play and three (3) additional cots stored in the storage unit on the side of the house. Bedroom #1 is used for changing diapers. LPAs observed 8 chairs, one (1) table, and four (4) high chairs that were safe, clean, and appropriate for the ages of the children. PAGE 1 OF 5 LPAs observed the children will have access to bathroom #1 inside bedroom #1. The vanity cabinet and a side cabinet that holds paper products, personal items, no hazards were observed, and the cabinet is secured with the use of a child proof latch. LPAs observed that the bathroom that children will use was safe and sanitary. The outdoor play area is located on the back yard of the house, is fenced and there are no hazards to children present. The children have access to a climbing structure that is in need of repair (photo taken) and has caution tape on the areas that need repair. Licensee stated that the climbing will be demolished by August 18, 2025 and will be replaced. LPAS observed there are toys and play equipment that are free of sharp pointed parts, in good repair, and appropriate for the ages of the children. LPAs also observed off limits to children and parents a storage that is made inaccessible with the use a door lock and an outdoor kitchen with two (2) mini refrigerators that are made inaccessible with the use of blocking entrance to kitchen and child latches on the doors. Licensee stated that when a child shows signs of an illness the child will isolated in bedroom #1 away from the other children. The combination dining/kitchen/ laundry room, bedroom #2, bedroom #3, bathroom #2, and are off-limits to parents and children and made inaccessible by use of safety gate between the living room and dining room that blocks the entrance to those areas. There is telephone service via a cellphone. LPAs advised Licensee that the cellphone must remain in the child care home during the operation hours. There is a combination smoke detector and carbon monoxide detector in living room; and was tested and is operable. There is a working fire extinguisher, the valve on the required 2A:10BC fire extinguisher indicates fully charged and Licensee confirmed purchase date was 04/15/2025. LPAs did not observe a fireplace or any open-faced heaters. There is adequate heating and ventilation for safety and comfort. LPAS did not observe any pools, spas, hot tubs, fish ponds, or similar bodies of water during the inspection on the property. PAGE 2 of 5 Per Licensee, there are no firearms or ammunition or other weapons on the premises. LPAS did not observe any firearms or other weapons on the premises. Per Licensee, there are no poisons in the home and LPAS did not observe any during the inspection. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible with the use of a safety latch. Licensee stated there are two (2) dogs on the premises. Licensee stated that during the hours of operations the two dogs are kept bedroom #3. There are currently no infants in care under the age of 12 months. LPAS advised that infants are checked on every 15 minutes and the time of each 15-minute check was documented with child\342\200\231s name and date. A LIC 9227 Individual Infant Sleeping Plan for each infant up to 12 months of age and reference Provider Information Notice (PIN) 20-24 CCP. LPAS discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAS also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was reminded that children in care are to be supervised at all times and made aware that children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. Capacity as specified on the license is being maintained. LPAS reviewed a sample of children\342\200\231s files and observed files are complete and documented on the LIC857. LPAs reviewed a personnel file and observed that the files was missing pertussis and measles vaccination verification. PAGE 3 OF 5 An emergency fire/disaster drill has been completed within the last 6 months on 06/06/2025 and the documentation of the fire/disaster drill was not readily available upon request. Licensee\342\200\231s Mandated Reporter Training was completed on 04/09/2025 and a completion certificate was available upon inspection. Licensee\342\200\231s pediatric CPR/First Aid expired on 04/2027. All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPAS completed the RSO profile in FAS. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to comply with posting requirement will result in an immediate civil penalty of $100.00. Exit interview was conducted with Liliana Cortez, Licensee including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. PAGE 5 OF 5
