What the inspector wrote

Licensing Program Analysts (LPAs) Lilia Hernandez and Daniel Bombino conducted an unannounced case management inspection on 08/04/2025 due to a self-reported incident that occurred at the facility. LPA arrived at the facility at 10:35AM and met with Meredith Lemus Berganza, Director, and Dalilah De La Cruz, Assistant Director, who guided LPAs on a tour of the facility. There were 44 children in care and 8 staff present upon arrival. The purpose of the visit was to follow up on an incident that occurred on 06/04/2025 at the facility. Information reported to the Department indicated that Child #1 and Child #2 engaged in an activity that was not appropriate. On 07/23/2025, LPAs conducted interviews with Assistant Director, Staff #1,Child #1 and obtained written statements for this incident. During today's visit, LPAs interviewed Child #2 and Parent #1. Based on information obtained, there were no deficiencies at this time. There were no deficiencies cited during today’s inspection. The Notice of Site Visit was posted and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted and report was reviewed with Meredith Lemus Berganza, Director.
