On 5/14/25 at 1:29PM Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Plan of Correction (POC) Visit. Upon arrival, LPA disclosed the purpose of the inspection and met with\302\240Director Mary Kumar, who guided the LPA on a tour of the facility. There were 6 infants present during today\342\200\231s inspection being supervised by 2 staff. During the Required - 3 Year inspection conducted on 3/13/25, the following deficiencies were issued: Inspection Tool Notes: Physical Plant - Type B: 1597.16(a)(1) - Water has not been tested CLEARED Care and Supervision - Type B: 101230(b) - Sleeping infants are not kept in a disturbance free room to nap. CLEARED Care and Supervision - Type B: 101429(a)(2)(B) - 15 min checks are not being documented CLEARED Care and Supervision - Type B: 101429(a)(2)(B)(3)(a) - all infants are missing LIC 9227. CLEARED Care and Supervision - Type B: 101430(a)(1) - missing written activity plan. CLEARED Staff Records - Type B: 1596.7995(a)(1) - 3 of 5 staff do not have MMR, 4 of 5 do not have T-dap, 5 of 5 do not have Flu or declination records CLEARED Staff Records - Type B: 101216(g)(1) - 4 of 5 staff do not have record of TB, 3 of 5 staff do not have record of health screening. CLEARED Staff Records - Type B: 101216(l)(1)(B) - 5 of 5 staff missing LIC 9052 CLEARED Staff Records - Type B: 1596.8662(b)(1) - 5 of 5 staff do have or has expired mandated reporter training. CLEARED Children Records - Type B: 101221(a) - of 5 children files reviewed, 1 child did not have a file, of the remaining files, for admission agreement 1 was missing 1 was not complete, 4 missing LIC 701, 2 missing lic 995, 2 missing lic 627,2 missing immunizations, 2 missing lic 9227 and all infants did not have the needs and services plan. CLEARED Children Records - Type B: 101229.1(a) - 2 of 5 children were not signed in. CLEARED The deficiencies noted above have all been cleared and no further action is needed. Exit interview conducted and report was reviewed with Administrator Vasanthy William. A notice of site visit was given and must remain posted for 30 days.
