What Happened
The annual inspection found multiple significant compliance problems, including an adult in the home who was not cleared, missing staff and child records, and unsafe items in the home. The inspector also cited the licensee for lacking current CPR/First Aid and mandated reporter training, and for unsafe child-accessible areas such as bathroom disinfectants and a changing table on a play yard.
Observations
- Adult 1 was present but was not cleared to be in the home
- Bathroom disinfectants were accessible to children
- Changing table was placed on top of the play yard where the infant slept
- 15-minute infant sleep log was missing
- Licensee's CPR/First Aid had expired and mandated reporter training was expired
- Child file C3 was missing an immunization record

Based on observation Licensee did not meet above criteria with the play yard gates with observable thick layer of dirt. A large filled bag of trash in the outdoor play area next to toys. Bathroom floors wet with dark foot print stains. Cockroaches crawling on the sink bowl, and in the top drawer. Kitchen piled with food and cleaning solutions next to each other on the same shelf., the licensee did not comply with the section cited above in the overall facility which poses a potential health, safety or personal rights risk to persons in care.
