What the inspector wrote

On 8/12/2025, Licensing Program Analyst (LPA) Brittanee Cleveland conducted an unannounced required 3 year inspection to the above facility. LPA arrived at 12:36 PM, identified self and met with Rachelle Grisby, Director, who guided analyst on a tour of the facility. This is a preschool program which consists of one classroom. The Facility\342\200\231s operation hours are Monday through Friday from 8:00am to 6:00pm. All areas identified on facility sketch were inspected. Upon arrival, the following staff were present during this inspection: Rachelle Rigsby, Heidi Knapp, and Aradenia Aguilar Soto with 18 preschoolers. There was a volunteer observed upon LPA\342\200\231s arrival, Rommel Chevez, Director\342\200\231s husband. The facility was observed to be within the license capacity and limitations. The facility has electronic sign in/out procedures that follow licensing compliance. The staff to child ratio was in compliance. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubbies to store their belongings. Per director, the isolation area is in director\342\200\231s office, where books and activities are available for sick children if needed. Age appropriate sinks, toilets and stools were inspected for good repair in all restrooms. Disinfectants, cleaning solutions, medications and other items that are dangerous to children, were inaccessible to children. Director stated that all poisons are locked in a cabinet located under the kitchen sink. The kitchen is off limits to children. Carbon monoxide detectors were observed and are operable. All kitchen areas, food preparation areas, and food storage areas are kept clean and are free of litter, rubbish, rodents and/or any other vermin. All storage containers for solid waste have tight fitting covers and are in good repair. Trash cans used to discard food have tight fitting lids are discarded immediately after each meal. ---Page 1 Outdoor playground area is in safe condition, free of any sharp, loose, or pointed objects. The surface of the outdoor activity play area is in safe condition and is free of hazards. All areas around or under climbing equipment, swings, slides, and similar equipment are cushioned with materials to absorb any falls. There is adequate shade in the play yard. Availability of outdoor drinking water was observed to be in a two gallon jug. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free. There are no firearms stored on the premises. There are no pools or bodies of water on the premises. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal background clearance, exemption or transfer their existing clearance or exemption, prior to initial presence in the childcare center. Director and present staff are trained in CPR and Pediatric First Aid. Children\342\200\231s records were reviewed. The name address, and telephone number of children\342\200\231s authorized representative are on file. Inspection of required forms were made and documented on LIC 857. LPA also reviewed staff records. The review of staff records is documented on the LIC 859. Director and present staff did have proof of the mandated reporter training and immunizations on file. All staff have been given on the job training for sanitation, housekeeping, and universal health precautions. Children\342\200\231s roster was observed to be current. Disaster drill log was reviewed, last drill was conducted on July 25, 2025. The facility does provide AM and PM snack, which were recorded on the monthly menu that is also given to parents and displayed on welcome board. First Aid was observed on the wall next to kitchen entrance. According to director, there are no children that need regular medicine usage. After observing the facility, there were no deficiencies during the inspection. --- Page 2 To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . PIN 22-05-CCP Page Four For the following scenarios use the below un-bolded language: \342\200\242 \342\200\242 CCC COMPLETED TESTING AND IS IN THE PROCESS OF REMEDIATING LEAD EXCEEDANCES, or \342\200\242 \342\200\242 CCC COMPLETED SAMPLING BUT THE TEST RESULTS ARE NOT YET AVAILABLE, or \342\200\242 \342\200\242 CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE, or \342\200\242 \342\200\242 CCC TESTING DEADLINE HAS NOT PAST LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information . --- Page 3 LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
