What the inspector wrote

On 5/24/2023, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management incident inspection to follow up on an Unusual Incident reported to the department by telephone on 5/16/2023. LPA was greeted by Owner Deidre Nordel and Assistant Director, Sonia Portalatin. LPA toured the facility and took a census of the children. Upon arrival, there were 70 children and 15 staff present today. Description of the incident : Director reported on 5/16/2023 at approximately 12:00pm, C1 went to sleep before lunch time. S1 conducted a head count and walked outside to the lunch tables. As the class arrived to the lunch tables, S1 conducted a second head count and noticed all the children were not present. Director stayed with classroom while S1 went back inside the classroom to get C1. C1 was left in the classroom unsupervised. Type A citation is cited for Lack of Supervision and a civil penalty of $500.00 is assessed. See LIC 809D for deficiency. A copy of this report must be provided to the authorized representative of all currently enrolled children and any newly enrolled child for the following 12 months. The acknowledgement of receipt of Licensing Reports (LIC 9224) shall be signed and kept in each of the children’s records. The notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will results in a civil penalty of 100.00. An exit interview was conducted, and a copy of this report and appeals rights was provided to Assistant Director.

C1 was left in the classroom unsupervised which poses an immediate health, safety or personal rights risk to persons in care.
