What the inspector wrote

On 8/1/2023, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on an Unusual Incident, reported to the department by telephone on 7/13/2023. LPA was greeted by Director and toured the facility and took a census of the children. Upon arrival, there were 81 children and 15 staff present today at the facility. Description of the incident : On 7/12/2023 at approximately 3:45pm, during indoor play time C1 was reaching for a toy off the shelf and fell. C1 hit his right eye lid on the bottom of the shelf, causing a gash above the eyelid. Staff washed the eye area, applied ice and pressure to the eyelid. Parent was notified and C1 was picked up right away. Parent took child to medical provider and C1 received stiches. C1 return back to the facility with doctors note on 7/14/2023. During this inspection, LPA toured the facility, interviewed staff, obtained pertinent documents, observed child and inspected the classroom area. Based on the information provided and interviews conducted further investigation is required. An exit interview was conducted, a copy of this report and notice of site visit was provided to Assistant Director.
