On 9/13/2024, Licensing Program Analyst (LPA) Loyce Phillips, conducted a case management inspection to follow up on an Unusual Incident reported to the department by telephone on 9/6/2024. LPA was greeted by Assistant Director, Lydia Lemus and toured the facility and took a census of the children. Upon arrival, there were 55 children in care with 16 staff present. Description of the incident : On 9/5/24 during outside playtime, the children were on the playground. During transitional time from outside to inside, staff directed all the children to clean up. Staff 1 (S1) noticed Child 1 (C1) and Child 2 (C2) under the play structure. C1 and C2 had their pants down. C1 was laying down and C2 was sitting on top of C1. S1 immediately told both children to come out of the area and redirected the children to join the other children in their class. Both parents were notified. During this inspection, LPA toured the facility, inspected and took photos of the play structure area. LPA also interviewed staff, obtained a copy of the facility roster, a copy of the unusual incident report, children sign-in/out sheet for 9/5/24 and staff roster. Director, Emily Walton joined the visit and toured the play structure with LPA. Based on the information provided and interviews conducted the incident will require further investigation. An exit interview was conducted, a copy of this report and notice of site visit was provided to Director.
