Inspector’s narrative

What the inspector wrote

During the interview with both Directors, the Directors denied staff allowing Child 1 to use the restroom unsupervised, Staff not assisting Child 1 with toileting needs in a timely manner and Staff 1 pinching Child 1. Both Directors stated that children are never left unsupervised and while children are using the restroom, there is always a staff member waiting in front of the bathroom (with visual supervision) or waiting inside of the bathroom. In reference to the allegation of Staff not assisting Child 1 with toileting needs in a timely manner, both Directors denied the allegation to be true. The Directors disclosed that although Child 1 was fully potty trained and independent, staff were exceptionally attentive with Child 1 and complied with all requests made by the parent to accommodate the needs of Child 1. Staff created a designated bathroom chart specifically for Child 1 and assisted Child 1 during bathroom breaks. Staff also checked on Child 1 throughout the day to ensure Child 1 remained clean and dry. In the event of an accident, Child 1 was changed and the soiled items were sent home in a zip log bag. The Directors disclosed that Child 1 never showed any signs of discomfort or mentioned feeling uncomfortable while at the program. In regards to the allegation of Staff 1 pinching Child 1, both Directors denied the allegation to be true. The Directors described Staff 1 as being compassionate, sweet, soft spoken and calm when working with children. During the interview with staff, staff made no disclosures about observing any of the allegations stated above and denied the allegations to be true. The statements obtained were consistent and corroborated with the statements obtained from both Directors. During the interview with Staff 1, Staff 1 denied ever pinching Child 1 and described Child 1 as being smart, independent and nice to others. During the interview with children, children disclosed that they are not left unattended while using the restroom and that there is a teacher supervising them during bathroom breaks. The children did not present concerns and were happy with the care provided by staff and particularly described Staff 1 as being their favorite teacher. Parents interviewed did not present concerns related to the above-mentioned allegations and were pleased with the services and care being provided to their children. Pg. 2 of 3 Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted and report was reviewed with Ashley Indzhyan and Mariam Khachatryan and Appeals Rights provided. Pg. 3 of 3
