Investigator T. Brunelli from the Investigations Bureau also conducted interviews with the Reporting Party, Licensee, and staff. LPA received no pictures of bruising on child. LPA did receive a picture of glass but LPA was unable to determine the origin of the glass. Through interviews, LPA was unable to meet the preponderance of evidence to prove the allegations occurred nor was LPA able to find substantial evidence to show that the allegations were false and could not have happened. Therefore, the allegations are Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the allegations occurred. An exit interview was conducted with Director Ashley Accosta, CEO Denise Tyner attended via phone call. A copy of this report was read and discussed with Director Accosta and CEO Tyner. A copy of this report along with the Notice of Site Visit will be provided to Director Accosta. The Notice of Site Visit is to be posted in a prominent area for 30 days.
