What the inspector wrote

On 6/26/2024 Licensing Program Analyst's (LPA), Suzette Ornelas and LPA Amelia Morales conducted an unannounced Annual Required Inspection. LPA's were met by Licensee, MICHAEL TOROSSIAN , who guided LPA's on a tour of the home. Days and hours of operation are Monday through Friday 8a-5:30p. LPA toured the home inside and outside and a census was taken. LPA's observed 9 day care children and 2 adults. Capacity as specified on the license is being maintained. Family members residing at facility are: 1 adults and no children. This is a single family home with one room, kitchen and bathroom. There are two houses on the same lot with separate addresses, 5133 and 5135 Fulton Ave., Sherman Oaks, 91423. The house with address of 5135 Fulton Ave. is a licensed day care facility (License # 197492910). Both houses will share the same outdoor area. The applicant was advised that the day care children from both facilities cannot be commingled in the back yard. Children will eat and sleep in the living/dining room. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. The First Aid Kit was observed and complete. Per LIS the facility annual fees are current. The facility roster was observed, and current. There are age appropriate toys and napping equipment on the premises. Per licensee, mats/cots will be purchased in the near future due to growing children. The required fire extinguisher (2A-10BC) was serviced/purchased on 6/24/2024. Carbon monoxide detectors and smoke detectors are in operable condition and tested by licensee. Facility provides daily meals for the children. Licensee has posted as required the License, and all other required postings in a visible location. Per licensee, outside patio area or the couch near the restroom will be used as the isolation area. There are no firearms or ammunition on the premises. Home does not have a fireplace. Home does not have a pool or bodies of water. There is currently 1 infants in care. LPA's provided safe sleep information. Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. LPA's reviewed 5 children's records and observed records were incomplete. Licensee will provide LPA with copies of Immunization records for 4 children in care. An emergency fire/disaster drill has been completed and documented within the last 6 months. Licensees Pediatric CPR/First Aid is current (2/14/2023). A review of records indicates that all employees and/or volunteershave immunization records on file.. Licensee has completed the Mandated Reporter Training within 2 years (12/20/2022); however, assistant will complete mandated reporter training every 2 years moving forward. Licensee will provide LPA with proof of completion via email. All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPAs and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 2 Type B deficiencies are being cited. (see LIC809-D) This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee, MICHAEL TOROSSIAN
