What the inspector wrote

On 2/6/25, at 3:30PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. There were 28 children with 7 staff observed in the infant program. During cite visits conducted at the facility on 9/26/24, 11/13/24 and 2/6/25, LPA discovered that the facility does not have an assistant present on duty. According to Title 22 Regulations Section 101415.1 (a) / Assistant Infant Care Center Director Qualifications and Duties, In addition to the director, an assistant director shall be present and on duty if the center has 25 or more infants in attendance. This poses a potential health, safety or personal rights risk to persons in care. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Nassima Mojabi, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.

    BUILDINGS AND GROUNDS

    Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidenced by: Statements obtained from the Director and Staff corroborated that napping equipment and baby gates were in disrepair. During the visit conducted on 9/26/24, LPA toured the inside/outside of the facility and observed three cribs in disrepair. The cribs were missing wheels and were tilted/leaning on one side. Also, multiple baby gates located in the outside play area were broken and had been taped together with duct tape (pictures taken of the cribs & baby gates).

  • INFANT CARE TEACHER QUALIFICATIONS AND DUTIES

    Infant Care Teacher Qualifications and Duties. To be a fully qualified infant care teacher, a teacher shall have the following: Completion, with passing grades, of 12 postsecondary semester or equivalent quarter units in early childhood or child development education at an accredited or approved college or university. At least three of the units required in (c)(1) above shall be related to the care of infants or shall contain instruction specific to infants. This requirement was not met as evidenced by: During interview with staff, it was confirmed that unqualified staff are working at the facility as teachers. Staff members 2, 3 and 4 were named as staff working as teachers at the facility. During the visit conducted on 11/13/24, LPA reviewed staff files and confirmed that staff members 2 – 4 are not qualified teachers and did not have the 12 core semester units in early childhood or child development education and 3 semester units related to infant care. Staff 5 was missing transcripts. This poses a potential health, safety or personal rights risk to persons in care.
