What Happened

This was an unannounced case management follow-up to the prior annual inspection. The facility was out of ratio when the analyst arrived, and the visit also documented unsafe crib modifications, a facility sign missing the license number, and an incomplete roster with missing child birthdates.

Observations

  • Licensee arrived after the visit started, which put the facility out of ratio.
  • Three of six cribs had wood extensions added to the sides, and older children were observed sleeping in the modified cribs.
  • The posted facility sign showed the name and phone number but not the facility number.
  • The roster was incomplete and did not include birthdates for the listed children.
