Inspector’s narrative

What the inspector wrote

Licensing Program Analysts (LPAs) Brittanee Cleveland and Samantha Barba conducted and unannounced inspection to the above facility on 01/07/2026. LPAs arrived at the facility at 1:31PM and met with Licensee, Sharon Oren, who guided LPAs on tour of the facility. There were 14 children and 2 staff. The purpose of the visit is to address deficiencies that were discovered during a subsequent visit conducted by the Department. At the time of inspection, Licensee did not have an up to date and completed roster of the children enrolled. LPAs advised Licensee that a current roster that includes children’s full name, date of birth, address, parent’s names and daytime phone number, and physician’s information is required in a family childcare home. Based on interview and observations, LPAs issued a Type B deficiency, as this poses a potential health, safety, and personal rights risk to children in care. LPAs have requested that Licensee send updated roster via email by 01/09/2026, the plan of correction due date. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Sharon Oren, Licensee.
