What the inspector wrote

On 11/4/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced case management inspection to follow up on an Unusual Incident, reported to the department by telephone on 10/31/2025. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 31 in total children present with 5 staff. Members. LPA met with Beth Motty Site Lead. According to the Telephone UIR on 10/31/2025 Per ECE Coordinator child was climbing on a cot and asked not to climb and child took off running and went into the office which is located inside Classroom A and hit the bottom of desk, injuring his left eye and lump in right side of eye. Side of face was clean with water and ice applied. Child\342\200\231s face was bleeding. Parents called and picked the child up. Child was taken to the doctor. I spoke to parent today and child is doing fine. No stitches but glue was administered to the injury. During the inspection, LPA toured the facility, interviewed staff, obtained a copy of Accident/ Incident, Sign in Sheets Master List of Participants( facility roster), LPA reviewed one file during the inspection. LPA reviewed the sign in sheets for 10/30/2025 for Class A. Based on the information provided and interviews conducted further investigation is needed. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Site Lead. Beth Motty.
