On 11/5/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced case management inspection to follow up and close out the Unusual Incident, reported to the department by telephone on 10/31/2025. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 29 in total children with 6 staff. LPA met with Beth Motty Site Lead. LPA explained to the Site Lead the nature of the visit, to continue with interviews. On 11/4/2025, LPA conducted interviews with the staff and obtained documentation. During the inspection today 11/5/2025 LPA toured the facility and interviewed staff and (C1) The staff that were interviewed stated they did witness the child running into the office. There was a total of three staff present on 10/30/2025 in Classroom A. LPA concluded this case management incident visit. No Title 22 violations have occurred, there was no lapse in care and supervision, and no deficiencies cited. A notice of site visit was given and posted for 30 days.
