What the inspector wrote

On 12/15/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced case management inspection to follow up and close out the Unusual Incident, reported to the department by telephone on 12/5/2025. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 25 in total children with 9 staff. LPA met with Norma Izucar Site Supervisor . LPA explained to the Site Supervisor the nature of the visit. According to the UIR Parent was called and picked up child. Child was taken to emergency. Child returned to school 12/5/2025 with 2 stitches. During the inspection LPA toured the facility and interviewed two staff and one child. LPA Whitmore obtained a copy of the facility roster and Accident/ Incident Report. Based on information obtained and interviews conducted. ( S1) was with another child and observed what happened. When she looked up ( C1) was in the kitchen area with two other friends. When she looked up again.( S1) observed ( C1) slipped. First aid was administered.. Incident report was completed, LPA concluded the Case Management Incident Visit. No Title 22 violations have occurred, and no deficiencies cited. A notice of site visit were given and posted for 30 days. Exit interview conducted and report was reviewed with Norma Izucar, Site Supervisor.
