What the inspector wrote

On 01/29/2026, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced case management inspection to follow up on an Unusual Incident, reported to the department by telephone on 01/09/2026. LPA toured the facility and took a census of how many children and staff are present today. LPA observed 26 children present with 6 staff. Members. LPA met with Norma Izcuar, Site Supervisor.. Description of the Incident: ( C1) was playing with peers in the house area out doors. When he was hit accidentally with blocks. This prompted( C1) to get out of the brown house area. As he started to run out, he forgot there was a ledge of the brown house. This caused ( C1) to hit his mouth on the brown house ledge. There was a small amount of bleeding from the upper lip. ( C1) mouth was rinsed with cold water and a ice pack applied. Parents were contacted and mother took the child to the dentist. Mother called in the afternoon around 4:00p.m. to let the Supervisor know that she took the child to the dentist. ( C1) is ok. Mother attempted to bring ( C1) to school 01/12/2026.( C1) was turned away because a doctor's not was needed. During the inspection, LPA toured the facility, interviewed staff ( C1) was not present. LPA obtained a copy of Accident/ Incident Report & Doctor's note from the dentist. Based on the information and interviews with documents provided,further investigation is needed. A notice of site visit was given and posted for 30 days. No deficiencies cited. Exit interview conducted and report was reviewed with Norma Izucar, Site Supervisor.
