On 10/22/2025 at 1:30PM, Licensing Program Analyst (LPA) Angela Luz met with Licensee Hasmik Yepremyan to deliver an amended report that was originally delivered on 10/14/25. Also present at the time of inspection were 13 children (11 preschool, and 2 school age), assistant who is fingerprint cleared and associated to the facility, and one art instructor. The art instructor is present for one hour each week and is not left alone with children in care. The art instructor is exempt from requiring background clearance at this time. The amended report was to assess a civil penalty and add the name of Staff 3. Licensee understands to wait for an invoice and the instructions for payment. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee Hasmik Yepremyan.
