What the inspector wrote

On 09/16/2022 , Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced visit at this facility for the purpose of delivering the investigation findings regarding the incident that occurred at the facility on 04/22/2022. Upon arrival, LPA met with Licensee, Anahit Oganesyan and observed 1 children. The Department received the Unusual Incident Report (UIR) via phone call on 04/25/2022. According to the report, on 04/22/2022 at approximately 05:30 PM, Licensee\342\200\231s husband attempted to put a leash on the dog. The dog started barking and the leash slipped and the dog bit child\342\200\231s hand and back. The child had puncture wounds on left hand and back. Licensee stated she put ice and placed alcohol on wounds. Licensee contacted the child\342\200\231s mother and child was taken to the hospital. On 5/5/2022 a Case Management visit was conducted regarding the UIR submitted on 04/25/2022. During this visit, LPA observed 1 child being supervised by Licensee. LPA interviewed Licensee and obtained the following documents: Licensee's written Declaration form, Child Care Facility Roster (LIC 9040), and vaccination records for the French Bulldog and Chihuahua Mix. LPA also toured the backyard and other areas that are accessible to day care children. Further investigation was needed. The incident was forwarded to the Department\342\200\231s Investigation Bureau (IB) and it was subsequently investigated by Investigator Olivia Spindola who conducted interviews and obtained medical reports. Based on the available information obtained throughout the investigation, it was revealed that Licensee failed to provide proper care and supervision and violated child\342\200\231s personal rights when the licensee\342\200\231s husband brought the dog out from an enclosed space and to an outdoor space where children were playing. As a result, the child sustained serious bites and puncture wound injuries while in care. Furthermore, Licensee did not seek medical treatment or call 911 at the time of the incident. [CONTINUE ON PAGE 2] PAGE 2 The facility was issued two Type A citations in violation of 102423 (a)(2) Personal Rights. See LIC 809-D for deficiency page. A Civil Penalty Assessment (Immediate for Type A deficiency that resulted in Serious Injury) in the amount of $2,000 shall be assessed today, 09/16/2022 . Licensee was informed that a Supervisory Conference will be scheduled with the El Segundo Child Care North Regional Office. Upon receipt of the Type A Violation(s), licensee shall post the report for 30 days in addition to the Notice of Site Visit, provide copies of the licensing report to parents/guardians of children in care at the facility and obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file by the close of business the following day or the next day child returns to the facility. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. The Licensee was provided an LIC 9224 and informed to print additional copies from the CCL website. An exit interview was conducted and a copy of this report was provided along with LIC 9213 Notice of Site Visi and Appeal Rights to Licensee, Anahit Oganesyan.
