On 06/06/2025 at 12:30p.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 05/21/2025 and was reported to the Regional Office. LPA met with Aida Escobar, Site Supervisor and informed the nature of the visit. LPA observed 49 Children in care with proper teacher/child ratios observed. There was a total of 14 staff. According to the UIR mother of Hazel Bartolo informed the teacher on 05/22/2025 that she had taken her daughter to the dentist due to an injury at school the day before 05/21?2025. Hazel was running up the stairs to the sliding structure. She missed a step and fell forward, hitting her mouth on the steps her bottom lip was cut and bleeding. During the investigation LPA Whitmore interviewed ( S1) ( S2) ( S3) ( S4) & ( C1) LPA Whitmore obtained a copy of Incident Report and Personnel Record. At this time further investigation is needed. An exit interview was conducted and a copy of this report along with the Notice of Site Visit was issued to Aida Escobar.
