On 08/27/2025 Licensing Program Analyst( LPA) Doris Whitmore, conducted a case management to follow up on an Unusual Incident reported to the department via email on 08/22/2025. The document that was emailed was the Lic 624 Form Unususal Incident Report. LPA met with Shaunee Breaux Education Coordinator, and informed the nature of the visit. At the time of the visit there were 26 children in care and 6 teachers. On 08/22/2025 the on duty analyst received the LIC 624 via email stating that on 08/18/2025 during drop off, the parents of ( C1) stated that ( S5) ( substitute teacher) called around 12:34 a.m. alleging that the injury their daughter substained on Friday August 15, 2025 was not cause by a classroom peer but another teacher in the classroom. ( S6) told the parent that she reported the incident to the Inglewood Police Department. Parent communicated with the coordinator Shaunee' Breaux in the morning. Coordinator reported concern to management. Statements were taken from other staff at the site. Crystal stairs is investigating allegations by substitute teacher which includes gathering statements from staff and other pertinent information. LPA Whitmore obtained a copy of the incident report. Based on the investigation which included interviews with staff and one child, further information is needed. An exit interview was conducted with Shaunee' Breaux" copy of this report and notice of Site Visit was provided. LPA Whitmore informed Shaunee'Breaux that the Notice of Site Visit must be posted for 30 days.
