What the inspector wrote

On 12/11/2025,  Licensing Program Analyst, LPA, Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 10/23/2025. LPA Chavies toured the facility and met with site supervisor, education coordinator and Education Manager. There 18 were children being cared and supervised by 4 staff members. Incident Details: RP states C1 was identified via doctor’s note to the center on 10/23/2025 by the child’s parents. C2 mother contacted the school on 11/3/2025, and told the school that she had a doctor’s confirmation that her child has hand foot and mouth disease. Also, on 11/3/2025, C3 contacted the school with doctor’s confirmation indicating that this child was confirmed to have hand foot and mouth disease. On 12/11/2025 LPA observed children in care, conducted interviews, received facility report, personnel roster and facility documentation. Based on interviews conducted and review of personnel and facility documentation, it appears the facility is adhering to the Health and Safety Code governing California. An exit interview was conducted, report was reviewed and provided sit supervisor , education coordinator and Education Manager. Notice of site visit was provided and must be posted for 30 days.
