What the inspector wrote

On 02/12/2026,  Licensing Program Analysts, LPA, Tyra Chavies conducted an unannounced case management visit- Deficiencies. LPA Chavies toured the facility and met with Aida Escobar, Site Supervisor. There were 23 children in care being supervised 4 by staff members. On 12/12/2025 LPA Chavies received camera footage via email from Education Manager, Dr. Terri Jonhson. On 12/31/2025 LPA Chavies reviewed camera footage where it was observed S1 physically picking up C1, placing C1 on couch and restrained him for a period of time. Based on review of camera footage, this facility is being cited a Type B. (Please see LIC 809-D) An exit interview was conducted, report was reviewed and provided to Site Supervisor. Notice of site visit was provided and must be posted for 30 days.
