What the inspector wrote

On 02/12/2026,  Licensing Program Analysts, LPA, Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 12/05/2025. LPA Chavies toured the facility and met with Aida Escobar, Site Supervisor. There were 23 children in care being supervised 4 by staff members. Incident Details: A staff member texted me on 12/04/2025 around 8:00AM to let me know the mother of C1 notified the center regarding a comment her son made about a teacher (sub teacher ) hitting him on the cheek because she was mad at him. On 12/11/2025 LPA observed children in care, received facility report, facility roster and personnel documents. On 12/11/2025 LPA conducted in person interviews with Education Manager, S#1 and S#2 On 12/11/2025 LPA attempted to conducted interview with P#1 On 12/31/2025 LPA reviewed video sent via email by Education Manager Based on observation, review of video and interviews conducted, it does not appear that the personal rights of the child was violated. An exit interview was conducted, report was reviewed and provided to Site Supervisor. Notice of site visit was provided and must be posted for 30 days.
