What the inspector wrote

On 10/15/2024 at 12:26p.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection due to an incident that occurred on 08/30/2024 and was reported to the Regional Office. LPA met with Aida Escobar Site Lead and informed the nature of the visit. LPA observed 33 Children in care with proper teacher/child ratios observed. There was a total of 10 staff. According to the LIC 624 Unusual Incident/ Injury Report mother of C1 notified the center on Friday August 30, 2024 at 9:23 a.m. that she was taking her son to the doctor because he was limping the day prior on Thursday, August 29, 2024 when she arrived to pick him up from school at 4:00p.m. Child was observed limping by a lead teacher and substitute teacher around 3:45p.m. on Thursday August 30, 2024 right before mom picked up child. Site Supervisor spoke to mom about their observations. During the investigation LPA Whitmore interviewed the Site Lead at the time that the incident happened was Marina De Honor. LPA was able to conduct interview on the phone. LPA Whitmore obtained a copy of the Doctor's note and Accident/ Incident . The child was absent from school today. LPA Whitmore called the mother and used Focus Language International, Inc. LPA interviewed the mom on the phone. LPA Whimore reviewed the doctor's note from the doctor. The chid did return back to school on 09/03/2024 with restrictions. On the doctors note the doctor did not specify the cause of the limping. During interviews the class was transitioning from outdoors to indoors. S2 noticed that the child was limping. S2 checked the child and did not see any marks or bruises. There are no violations or deficiencies. A copy of this report was issued to Aida Escobar along with the Notice of Site Visit.
