On 09/11/2025, Licensing Program Analyst (LPA) Doris Whitmore conducted a case management inspection to follow up on a Unusual Incident, reported to the department by telephone on 09/04/2025. The incident was reported to the Regional Office. LPA met with Norma Izucar and informed the nature of the visit. At the time of the visit there was 43 children and 9 teachers. According to the telephone UIR guardian reported that( C1) stated that teacher hit him. ( C1) pointed to teacher ( S1) but did not verbalize the teacher's name. LPA Whitmore toured the facility. During today's inspection, LPA conducted interviews with staff, child and parent. LPA obtained a copy of the Personnel Report and Child Plus. At, this time further investigation is needed.
