On 09/16/2025 Licensing Prgram Analyst Doris Whitmore conducted a follow up case management inspection regarding an incident that occurred on 09/03/2025. LPA Whitmore met with Site Supervisor, Norma Izucar. LPA observed 29 children present and 10 staff. On 09/03/2025, the facility reported an incident that the guardian of ( C1) stated that teacher hit him. pointed to teacher but did not verbalize the teacher's name? On 09/16/2025, LPA Whitmore continued with the interviews. Based on investigation which included interviews. There was insufficient evidence to determine that Personal Rights violations occurred. LPA Whitmore concluded this case management incident visit No Title 22 violations occurred, and no deficiencies cited. A notice of of site visit was given to and posted for 30 days. Exit interview conducted and report was read and reviewed with Site Supervisor, Norma Izucar.
