What the inspector wrote

On 05/01/2026 at 1:39 P.M. Licensing Program Analyst( LPA) Doris Whitmore, conducted a case management inspection to follow up on the Unususual Incident, reported to the department by telephone LPA Whitmore met with Site Supervisor Davida Brown and toured the facility. LPA observed 88 children and 19 staff. Description of the Incident: ECC coordinator is reporting that on 04/17/2026 child name on report was crawling in the indoor playground and crawled under and hit her head which caused small bleeding. Incident was reported to incident report. First aid was administer and parents were notified of incident. Parents were instructed to seek medical attention for a doctor's clearance. Child was back in care, doctor's clearance with no restrictions. LPA interviewed ( S1) and ( C1). LPA obtained a copy of the Accident/ Incident Report and Doctor's Note.( C1) returned back to school with no restrictions Based on the investigation, documentation, and interviews, there was no lapse of Care of Supervision and no disclosures made. LPA concluded this case management incident visit. No Title 22 violations have occurred, and no deficiencies cited. A notice of site visit was given and posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor, Davida Brown.
