On 11/14/2024 Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced year Annual/ Required at Montessori of Torrance (Infant). LPA met with Director, Chandrika Peiris and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday 6:30 AM to 6:00 PM. LPA Chavies confirmed the phone number is: (310) 214- 9141. There were 17 children in care being supervised by 6 Staff members. Breakfast and lunch are provided by the parents as well as AM/PM snacks. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility kitchen and storage areas, changing tables, cribs and floors in classrooms meet Title 22 Health and Safety regulations. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. The facility does have a 2A10BC fire extinguisher on the premises. The facility has one or more functioning carbon monoxide detectors/ fire detectors. (Combination) Capacity and limitations as specified on the license are being maintained. All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than four infants in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the childcare center director or fully qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility is using their full legal signature and record the time of day. LPA Chavies did discuss the with director the importance of nonprescription medicine being stored and labeled appropriately. PG1OF3 Drinking water is available both indoors and outdoors. Areas around climbing equipment have cushioning material to absorb falls. All materials and surfaces accessible to children are toxic free. The facility is free of flies and insects. CHILDREN'S RECORDS: LPA Chavies reviewed 8 children's files. All files are up to date and are in good condition. STAFF'S RECORDS: LPA Chavies reviewed 4 staff files. Most of the Staff records are up to date. One staff member has a valid CPR and First Aid Certification. LPA Chavies did advise director that per CCR regulations infant staff are required to have 16 of Health and Safety and are required to have immunization for the following: MMR, TDAP and influenza. The following were also discussed with the director: \302\267 When received, the licensee is to provide to parents/guardians accepting services and to new parents/guardians the following for 12 months: A copy of any licensing reports that document a Type A citation. The licensee shall post citation for 30 days and during daycare hours. \302\267 Individuals who are 18 years of age or older working in the facility must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately . Failure to obtain a criminal record background check clearances prior to initial presence in the facility will result in an immediate $500.00 dollar civil penalty and $100.00 more per day until licensee is in compliance \302\267 Commencing September 1 st , 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. PG2OF3 \302\267 New Immunization Requirement: Law enacted by SB 277, beginning January 1 st , 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint. \302\267 New Appeal Process : A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). Per Title 22, of the California Code of Regulations, the following Technical Violations were discussed. (See LIC 9102) An exit interview was conducted with Director, Chandrika Peiris. A copy of this report was read and given to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.
