What the inspector wrote

On 06/03/2025 at 8:44 a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection for an incident that occurred on 05/12/2025 and was reported to the Regional Office. LPA met with Keyla Romero Teacher and informed the nature of the visit. At the time of the visit there was 26 children and 9 teachers. According to the UIR Child was washing her hands and she told the teacher that her dad pulled down her pants and she did not like that feeling. Child was speaking directly to teacher and no other child was in the vicinity. Facility contacted CPS and made a report. LPA interviewed ( C1) ( S1) and ( S2) documents obtained during the visit were the following: LIC 624 Unusual Incident/ Injury Report, Jira, and Suspected Child Abuse Report( SCAR). A File review was conducted for ( C1) LPA Whitmore contacted West LA Office and left name, report number and contact information for the social worker. At this time the Case Management Incident needs further investigation. .An exit interview was conducted notice of site visit and appeal rights were issued to Teacher Keyla Romero. LPA Whitmore informed Teacher Keyla Romero that the Notice of Site Visit must be posted for 30 days. Notice of Site Visit was posted before LPA Whitmore left the facility.
