What the inspector wrote

On 05/19/2026 at 8:55 a.m. Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection for an incident that occurred on 05/06/2026 and was reported to the Regional Office. LPA met with Sandra Amaral and informed the nature of the visit. At the time of the visit there was 26 children in care supervised by and 9 staff. LPA toured the facility indoors and outdoors Description of Incident: While outside child tripped and hit chin on apparatus, Teacher Sonia cleaned chin due to scratch. Small amount of blood on child's chin. Teacher called mom. Mom had child picked up early. Based upon today's visit interviews conducted with 3 staff and one child. LPA was able to view the area where the child fell. Staff were able to show where the children begin walking from to the playground and exactly where ( C1) fell. There were two staff that were on the playground when the incident occurred The documents that were obtained were the following: Personnel Report, Unusual Incident/ Injury Report, Jaira, and two doctor's notes. Child was taken to the doctor and received four stitches under chin, ( C1) returned with a doctor's note and modified activity .Staff were in ratio at the time of the incident. Staff were able to show LPA Whitmore where the incident took place. Based on interviews and documentation there was no lapse of care and supervision before children lined up staff informed the children not to run. There was one staff in front of the line and the other staff in the back of the line. The facility followed the appropriate regulations for care and supervision. Staff responded immediately, first aid was administered, and parents were notified. There are no deficiencies An exit interview was conducted, copy of this report was provided and read.
