Inspector’s narrative

What the inspector wrote

On 8/10/2022 at 7:00 am, Licensing Program Analyst (LPA) Silva Garibyan conducted an announced inspection with Miriam Rodriguez for the purpose of an increase of capacity inspection for 11137 Landale Street, North Hollywood, CA 91602. The purpose of this increase of capacity visit is to ensure the standards for a Family Child Care Home are being met in accordance to California Tittle 22 Regulations and California Health and Safety Codes. There was one preschool child present at the time of the visit. Currently the licensee is licensed for a small FCCH with a maximum capacity of 8 children. The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. Per the licensee there has been no noted changes to the home or occupants since her last visit (Required-1 Year conducted on 7/27/21.), the facility remains in substantial compliance at this time. A fire clearance was approved by L.A City Fire Prevention Bureau on 7/26/2022. There are currently two infants enrolled in care. LPA discussed Safe Sleep Regulations with licensee. There is one crib or play yard for each infant in care, cribs and play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard. Infants are not swaddled while in care. Provider physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Infants can be visually observed through an open door if sleeping in a separate room. Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping. Capacity and Ratios: The licensee has applied for a capacity increase for a large family . The Maximum capacity is 12 with no more than 4 infants and a qualified assistant. Without a qualified assistant the license reverts back to the requirements for a small family childcare. Optional care may be provided for a maximum capacity of 14 children with no more then 3 infants, 2 school age children and a qualified assistant. School age children must be 1 age 6 yrs and 1 in Kindergarten. No deficiencies were cited. Facility is in substantial compliance at the time of the visit.
