Inspector’s narrative

What the inspector wrote

On 3/11/25, Licensing Program Analyst (LPA) V. Wheatley conducted an inspection regarding the above allegation. LPA arrived at the facility and learned it is closed for the week due to staff training. LPA met with Director of Operations, Christopher Nwani who arrived at the facility. The licensee/director is unavailable today. LPA did not observe any children on the premises. LPA interviewed Mr. Nwani regarding the above allegation who admitted that a day care child was left on a bus after the children returned from a field trip on February 28, 2025. The child was found safe. Mr. Nwani states that two staff members were responsible for the child being left on the bus and they were suspended for 3 days without pay. Mr. Nwani also informed the LPA that the staff are all taking training this week regarding proper supervision which must be completed before they return to work on Monday, March 17, 2025. The facility is being cited a Type A violation for Lack of Care and Supervision. See LIC 9099D. The facility is being assessed a civil penalty for absence of supervision.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229(a)(1)-Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, This was evidenced by: During an investigation and admittance, a preschool child was left unsupervised on a bus during a field trip on February 28, 2025. Although the child was found safe, this is a Type A violation for lack of supervision which is an immediate risk to the health and safety of the children in care.

  • REPORTING REQUIREMENTS

    101212-Reporting RequirementsEach licensee or applicant shall furnish to the Department reports as required by the Department including, but not limited to, the following: (d)Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. This was evidenced by:The licensee/director failed to call Community Care Licensing Division within 24 hours of the incident happening and did not submit a written report to the Department within 7 days.
