What the inspector wrote

On 02/09/2026, Licensing Program Analyst (LPA) Judy Laureano arrived at the above mentioned facility to investigate the referenced allegation. LPA met with Facility Director Danielle Williams. LPA conducted a tour of the facility and observed 32 children, 6 staff members, and Director/Owner D. Williams providing care and supervision. Present during today’s inspection was administrative staff C. Nwani. A case management inspection was previously conducted on 02/02/2026 to follow up on a self reported unusual incident received by the Department on 01/20/2026, regarding an allegation that a staff member struck a child with a shoe. Based on a review of facility records, video footage, and staff interviews, a Type A citation was issued on 02/02/2026 for violation of CCR 101223(a) – Personal Rights. Therefore, the allegation that staff hit a day care child is SUBSTANTIATED. Exit interview was conducted with Director and a copy of the report and Appeal Rights were provided with a Notice of Site Visit.

    §1596.7995(a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidence by: LPA Laureano observed 1 out of 3 files incomplete, proof of influenza/flu vaccine and/or waiver was not available for review.
